docs: add Contributor Covenant 2.1 and issue templates

Adds CODE_OF_CONDUCT.md (Contributor Covenant v2.1) and
.github/ISSUE_TEMPLATE/{bug_report,feature_request}.md so contributors
have a clear, conventional way to file issues. Also unlocks the
"Community health" indicators on the GitHub repo page.
